Here is where you register to play in the Australian Senior Eight-Ball Championships or Australia Cup Eight-Ball Championships, after you have been selected in a team or offered a spot in the singles. Managers must also complete this form.

• Comply with any anti-doping requirements (as detailed on the next page).

This must be completed by each player on the team. This cannot be done on behalf of another player.
