Here is where the parent or guardian registers a player to play in the Australian Junior Eight-Ball Championships, after they have been selected in a team or offered a spot in the singles.

This must be completed by a parent or guardian for each player on the team. This cannot be done on behalf of another player.

The first few pages will contain some important notes for you to read before we can take your nomination.
